WHAT IS ‘PROOF OF INCOME?’

Proof of income is a document that shows how much money you make. It’s used by organizations like Portage Path Behavioral Health to determine out if you qualify for financial help. Here are some common documents that can be used as proof of income:

• Pay Stubs
• Tax Returns
• Social Security Benefits Statement
• Unemployment Benefits Statement
• Disability Benefits Statement
• Retirement/Pension Income
• Self-Employment Income
• Bank statements

By providing proof of income, you enable us to verify your eligibility for these subsidized services, ensuring you receive the care you need without the burden of high costs.

Portage Path, partially funded by Summit County ADM Board, provides additional services free to residents of Summit County who are not Medicaid eligible. To qualify for these free services, the ADM Board requires income documentation. By providing proof of your income, you automatically become eligible for these subsidized services.

HOW TO SUBMIT PROOF OF INCOME

You can submit proof of income in these ways:

In Person: stop by and bring your documents to any of our offices during normal business hours

Email: take a picture on your cell phone and email your documents to our secure email address.

Scan: scan and email copies of your documents to our secure email address.

Mail: mail copies of your documents to your office’s address below

Fax: fax copies of your documents to your office’s fax number listed below

Once you’ve submitted your proof of income, you will automatically have access to these additional services at no cost to you:

  • Case Management Services Includes:
    • Addressing life needs like affordable housing, food banks, or transportation
    • Linking with needed medical, social, educational, or spiritual services
    • Assisting with courts, legal issues, governmental or employment agencies
    • Providing support for clients while they get their lives back in order
  • Pathway Program:
    • Learn how to cope with difficult situations.
    • Learn how to manage and express emotions.
    • Improve your understanding of trauma and its impact on symptoms, relationships, and overall functioning.
  • Crossroads Program:
    Dual diagnosis services such as an Intensive Outpatient Treatment Program and Support Groups that are designed to help those experiencing mental illness and substance use disorders to take control of their recovery and return more quickly to their best possible life, with fewer relapses.
