Job Overview
We are seeking a highly skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Primary Care Team. This role involves delivering high-quality patient assessments, managing acute and chronic conditions, and supporting our mission to provide effective, affordable mental health and medical services within a community setting. This position is contract, seeking a minimum of 8 to 16 hours of weekly coverage (Monday – Friday, no holidays and no weekends).

Key Responsibilities

  • Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions
  • Conduct physical exams and obtain medical histories
  • Order and interpret laboratory and diagnostic tests
  • Prescribe medications and treatments in accordance with state and federal regulations
  • Provide preventive care including wellness exams, screenings, and immunizations
  • Manage chronic diseases such as diabetes, hypertension, and asthma
  • Educate patients on disease prevention, lifestyle modifications, and medication adherence
  • Collaborate with physicians, behavioral health providers, dentists, and care coordinators in a team-based care model
  • Document care accurately in the electronic health record (EHR)
  • Participate in quality improvement initiatives and population health efforts

Qualifications

  • Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
  • Current state licensure and national certification
  • Current DEA license or eligibility
  • Current BLS certification
  • Strong commitment to serving medically underserved populations
  • Experience in family medicine, community health, or primary care (preferred)
