Portage Path is currently seeking a full-time Case Management Professional that is highly skilled in working with clients with Co-occurring Substance Use (SUD) and Mental Health (MH) Disorders. The SUD Case Manager reports directly to the CPST Team Leader and works in conjunction with multi-disciplinary teams to assist in meeting the needs of clients of the Portage Path’s Dual Diagnosis services and programming. Compassion, experience and knowledge of the challenges of dually diagnosed clients are essential to the responsibilities of the position. The SUD Case Manager/CPST must have knowledge of mental health and substance use symptoms, disorders, and diagnoses. Must possess knowledge of community resources and programs, and skills to make linkages and referrals. Responsibilities include determining the level and type of services necessary for clients to improve and function effectively in the community while providing direct-care case management services. The candidate must demonstrate the ability to assess and monitor clients’ levels of need, client symptoms, and functioning. The SUD Case Manager is responsible for assisting clients to secure necessary resources, links to community services, in addition to coordinating client’s treatment plans. The successful candidates must have a Bachelor’s or Master’s degree in a related field. In addition, the candidates must possess excellent organizational and communication skills with the ability to work within a multi-disciplinary team. Reliable transportation is required.
