Care Navigator

Portage Path Behavioral Health has a New and Unique opportunity for a Care Navigator. The role of the Care Navigator is to assist Summit County residents that are hospitalized for psychiatric care, connect to community care upon discharge. The position requires strong communication skills to establish solid working relationships with local and regional hospital staff, agency staff, The County of Summit Alcohol, Drug and Mental Health Service Board, and community partners. The Care Navigator will work collaboratively with community partners and work closely with clinical and administrative staff at local agencies to track and encourage treatment engagement and compliance. Additional job duties may include completing initial admission assessments in a hospital or community setting. The successful candidate will be licensed as an LPC / LPCC, or LSW / LISW and hold a valid Ohio driver’s license with reliable transportation to travel to hospitals and settings both within and outside of Summit County.
