Our Care Navigator is an exciting new position at Portage Path Behavioral Health designed to improve Summit County behavioral health clients’ connection to ongoing mental health and/or substance use treatment, increase client engagement and accessibility, enhance the overall quality of client care, and reduce the number of hospitalizations by providing better community-based stabilization for individuals. The Hospital-based Care Navigator will work primarily at Summa and Cleveland Clinic Akron General’s Inpatient Psychiatric Units and interface with the hospital treatment team. When Summit County Residents are receiving inpatient psychiatric care at other hospitals throughout Ohio, the Hospital Navigator will also communicate with those providers and coordinate care. The Care Navigator will report to the Crisis Coordinator and Vice President of Psychiatric Emergency Services and work collaboratively with the Care Navigator from Community Support Services (CSS) to ensure coverage, care coordination and tracking of Summit County Clients. Candidates must be licensed as an LSW/LISW or LPC/LPCC with the ability to assess and render preliminary diagnoses according to DSM5/ICD10 standards. The position requires a valid Ohio Driver’s license and an ability to travel to hospitals and settings both within and outside of Summit County.
Care Navigator Responsibilities:
- Effectively address the needs of individuals/families in Summit County and establish solid working relationships with local and regional hospital staff, agency staff, The County of Summit Alcohol, Drug and Mental Health Service Board, and community partners.
- Work directly with Psychiatric Emergency Services (PES) and hospital treatment staff to gather information and provide support to individuals and families in-person, via telephone or video conference.
- Connect individuals with mental health and/or substance abuse treatment.
- Complete intake assessments prior to hospital discharge to expedite process of connecting individual to outpatient mental health and/or substance abuse treatment.
- Conduct follow up contacts to ensure safety, reinforce treatment recommendations, and/or build support for success.
- Collaborate with community partners as well as clinical and administrative staff at local agencies to track and encourage treatment engagement and compliance.
- Gather and analyze data and outcomes of the project.
Care Navigator Requirements:
- Clinical licensure in the State of Ohio.
- Valid Ohio driver’s license.
- Reliable transportation.
- Strong verbal and written communication skills required.
- Experience in crisis services and substance abuse field is preferred.
