Medicaid to Require Verification of Citizenship
As part of the Deficit Reduction Act of 2005, all persons submitting applications and re-determinations of eligibility for Medicaid after July 1, 2006 must provide proof of citizenship in the form of a birth certificate, passport, or certificate of naturalization. Individuals currently enrolled in Medicaid will lose their coverage if unable to provide one of these documents. Below is a State of Ohio resource that may be helpful in obtaining birth certificates or passports:

FOR CLIENTS: Change in paperwork/forms procedure
All PPBH clients in need of having paperwork or forms signed by a doctor or therapist or processed by PPBH should check with the reception desk to get a copy of our new procedure for handling such requests. While certain forms can be taken care of immediately (on the day of your appointment), some take 7-10 days to process, and others take up to 30 days.
Please be advised that it is much easier for us to take care of your paperwork requests on the day of your appointment.
